Community Health Worker in Dallas

A community health worker in Dallas from Focus2Impact walks beside you — helping you understand your health options, connecting you to free community resources in Dallas, and making sure no one has to navigate the system alone.

What a community health worker does
A community health worker (CHW) is a trusted neighbor with training in health education and local services. Our Dallas community health workers listen first, then help you find care, benefits, and support that fit your situation.
Because our CHWs come from the communities they serve, they understand the real barriers families face — transportation, paperwork, language, and cost — and know how to overcome them.
One-on-one health education and preventive care guidance
Help enrolling in benefits and finding low-cost clinics
Food and housing resource navigation for families in crisis
Connections to non-emergency transportation assistance in Dallas for medical appointments
Ongoing encouragement and follow-up, not just a one-time referral
Free health education and preventive care
Focus2Impact community health workers lead free sessions on wellness, chronic-condition prevention, and healthy living. We believe health education should be practical, judgment-free, and available to everyone — no insurance required.
Whether you need help understanding a diagnosis, finding a screening, or building healthier habits, a community health worker in Dallas is ready to help.

We serve underserved families, seniors, and individuals experiencing homelessness across Dallas. Every service is free, and every conversation is confidential.
